CentraState Healthcare System in Freehold, New Jersey, is seeking a dedicated Physical Therapist to join a multidisciplinary team providing inpatient and outpatient rehabilitation. The role emphasizes evidence-based practice, strong communication, and adaptable treatment plans for diverse patient populations.

The ideal candidate will have Doctorate or Master’s in Physical Therapy, active PT licensure, and CPR/BLS certification, with 1+ year experience preferred.

Qualifications

• Doctorate or Master's Degree in Physical Therapy from an accredited program.
• Current state licensure as a Physical Therapist (PT).
• CPR/BLS certification (or willing to obtain upon hire).
• 1+ years of experience preferred (new graduates encouraged to apply).
• Prior experience in both inpatient and outpatient settings is a plus.
• Strong organizational, communication, and interpersonal skills.

Responsibilities

• Evaluate, plan, and implement individualized treatment programs for patients in both inpatient and outpatient settings.
• Use evidence-based techniques to improve mobility, relieve pain, and prevent or limit permanent physical disabilities.
• Collaborate with physicians, nurses, occupational therapists, and other members of the care team.
• Educate patients and families on treatment plans, exercises, and post-care recovery.
• Accurately document assessments, goals, progress notes, and discharge summaries in EMR system.
• Maintain compliance with all regulatory standards, policies, and procedures.
• Participate in team meetings, continuing education, and quality improvement initiatives.
