Job summary

Capital Health seeks a licensed Physical Therapist to assess, evaluate, and plan patient-specific therapies across inpatient, outpatient, and home-based settings within the hospital network in New Jersey.

The role emphasizes evidence-based treatment, meticulous documentation, and collaboration with physicians, nurses, and care managers to optimize outcomes. A minimum of a PT degree and BLS/ AED training is required, with potential pelvic floor experience a plus.

Qualifications

• Graduation from accredited physical therapy school or education approved by NJ State Board of Physical Therapy Examiners.
• One year experience in acute adult inpatient, adult outpatient, or pediatric physical therapy preferred.
• BLS or AHA Heartsaver AED.

Responsibilities

• Evaluate and treat patients in the hospital, outpatient clinic and in the home under the hospital at home program.
• Coordinate care with physicians, nurses, case managers, and other CH interdisciplinary team members.
• Develop treatment plans and establish measurable goals for patients; implement treatment plans using appropriate procedures and modalities.
• Ensure timely and complete documentation of all aspects of care, including evaluation, care, attendance, and discharge.
