Job summary

Concord Hospital Health System is seeking an Inpatient Physical Therapist per diem to evaluate patients and provide treatments across ages and conditions. You will participate in indirect care as needed and help cover weekend shifts and holidays.

Flexibility to cover multiple Concord Hospital sites is required. Must have graduated from an accredited Physical Therapy program, possess a NH license in good standing, and maintain BLS certification.

Qualifications

• Graduated from an accredited Physical Therapy program.
• Must have passed the national certification board.
• Active NH Physical Therapy license in good standing.

Responsibilities

• Performs patient evaluations and re-evaluations and documents in accordance with licensure, scope of practice.
• Develops and documents assessments for each patient at evaluation and during treatment.
• Develops, revises, and documents goals and treatments for all patients during the episode of care.
• Demonstrates adaptability and timeliness in managing the department caseload.
• Maintains accurate and thorough patient charging as part of daily charting.

Education

Physical Therapy degree (DPT or equivalent)

State NH Physical Therapy license
