Qualifications

• Graduate from an accredited Physical Therapy program.

• Experience working with a multidisciplinary team and diverse patient populations.

Responsibilities

• Performs patient evaluations and re-evaluations and documents per licensure and department standards.
• Advocates for and develops disciplinary and interdisciplinary team collaboration.
• Develops assessments, goals and treatment plans for each patient.
• Maintains accurate charging and charting as part of daily workflow.
