The pharmacist will coordinate dispensing, review patient charts, monitor therapy, and provide drug information to healthcare staff, ensuring safety and quality of care in a hospital setting.

Qualifications

• Must hold a degree from an accredited college of pharmacy; residency in hospital pharmacy strongly preferred.
• Demonstrated verbal and written communication skills to disseminate drug information to health care personnel.
• Thorough understanding of pathophysiology and pharmacotherapy for proper monitoring of drug therapy.
• Experience in teaching programs including instruction of students, interns, and technicians.
• Ability to work with others to develop improved patient services.

Responsibilities

• Coordinates dispensing of unit dose, IV admixtures, investigational drugs and other pharmaceuticals.
• Identifies and monitors pharmaceutical needs of patients.
• Reviews patient charts/interviews to obtain information for medication records.
• Reviews drug information to identify therapy problems and recommends solutions.
• Verifies medication orders for completeness and legality; verifies prepared medications.
