Job summary

Shodair is seeking a Resource Patient Care Coordinator to support patients and families from admission through discharge. This role ensures a smooth intake process, initiates treatment planning, and coordinates interdisciplinary care to meet each patient’s clinical and emotional needs.

Qualifications

• At least 1 year of experience working with individuals with mental illness.
• 2+ years in case management, patient care, or social services (preferred).
• Experience in inpatient or acute settings is a plus.
• Proficiency with EPIC and standard office technology is required.

Responsibilities

• Coordinate intake and discharge processes for patients.
• Facilitate treatment planning with an interdisciplinary team.
• Support patients and families, ensuring clear communication and care coordination.
