Job summary

Shodair-Children in Helena, MT seeks a Float Patient Care Coordinator (PCC) to provide coverage for Acute, Residential and Resource Coordinators. You will coach patients in group settings, participate in 1:1 care coordination, and help develop and monitor interdisciplinary care with the treatment team.

The role involves leadership, advocacy, and liaison work with families and community professionals, as well as completing billing and documentation to meet standards.

Qualifications

• Post-secondary education or Bachelor's degree in a human services field strongly preferred.
• At least 3 year of experience serving people with mental illness is required.
• 5 years of experience in case management or patient care in a healthcare or social services delivery system preferred. 1 Year experience in a patient care setting.

Responsibilities

• The Float PCC does not have an assigned patient caseload; however, the position supports Acute, Residential and Resource Patient Care Coordinators with coverage needs.
• Provides skills coaching to patients in a group setting at least 4 times weekly and participates in 1:1 care coordination service.
• Coordinates care to patients by developing, monitoring, and coordinating interdisciplinary care in conjunction with the treatment team.
• Provides effective leadership and advocacy in building relationships between teams, families, and other community professionals.
• Completes their own billing and note documentation to comply with hospital and governing agency standards.
• Assists families in navigating community services and systems and can help in the application and referral process.

Education
