• Knowledge of admission, inpatient stay, transfer, discharge and ambulatory procedures.

Responsibilities

• Identify patients/families for Care Coordination services in a systematic manner.
• Coordinate with inpatient and outpatient teams to ensure timely care.
• Document conversations, referrals, and discharge planning.
• Support services such as social work, language services, and case management for seamless patient care.
