US Career Partners is recruiting a Nurse Practitioner or Physician Assistant to provide outpatient and SNF-based palliative care across Essex & Hudson counties in New Jersey. The role offers a structured ramp, physician backup, and a collaborative team environment.

Qualifications
- Active New Jersey Nurse Practitioner or Physician Assistant license with national board certification.
- DEA + CDS registration, or ability to obtain by start date.
- 2+ years of palliative care or closely related serious-illness experience.
- Palliative care certification (e.g., ACHPN / CHPN) preferred.
- Strong communication, clinical judgment, compassion, and comfort working on interdisciplinary teams.
Responsabilités
- Deliver palliative care consultations and follow-up visits across SNFs and community settings.
- Lead goals-of-care and advance care planning conversations with patients and families.
- Coordinate with home health and hospice agencies as appropriate.
- Complete timely documentation aligned with billing and quality standards.
