Job summary

NJ Palliative Care Solutions is seeking a Nurse Practitioner or Physician Assistant to provide hospital-based palliative care at Community Medical Center in Toms River, NJ. Inpatient role requires palliative care experience and NJ licensure.

Qualifications

• 2+ years of palliative care experience required.
• NJ NP/PA license and national board certification required.
• DEA + CDS registration or ability to obtain by start date.

Responsibilities

• Provide hospital-based palliative care consultations and follow-up visits across the hospital continuum.
• Deliver patient-centered, evidence-based palliative medicine with a holistic approach.
• Lead goals-of-care and advance care planning discussions with patients and families.
• Coordinate with SNFs/hospices when appropriate and integrate into hospital workflows.
• Complete timely documentation aligned with clinical, billing and quality standards.
• Spend time on discharge planning and transitions to next setting, honoring patient preferences.
