Job summary

NP Now in Findlay, Ohio is expanding its inpatient palliative care program and seeks a dedicated NP to work 75% at Findlay hospital and 25% at Lima hospital. The role is full-time, Monday through Friday, 8:00 a.m. to 4:30/5:00 p.m., with shared evenings and weekend phone calls.

Qualifications

• Certified Nurse Practitioner with licensure to practice in Ohio.
• Acute Care NP preferred; palliative care or hospice experience welcomed.
• Excellent communication and organizational skills.

Responsibilities

• Perform inpatient palliative care consultations and follow-up visits.
• Manage symptoms for patients with serious illnesses.
• Facilitate goals-of-care discussions and advance care planning.
• Assist with discharge planning and transitions to home-based care or hospice.
• Provide inpatient rounding for hospice GIP patients as needed.
