You will also oversee scheduling, 24/7 coverage, and on-call rotation while leading staff development and quality initiatives within regulatory requirements.

Qualifications

• Must have reliable transportation with valid license and insurance.
• Knowledge of primary medical and nursing care, with palliative care basics.
• Understanding of interdisciplinary team dynamics and the dying process.
• Two years supervisory experience in a health care organization.

Responsibilities

• Directs patient care services for the team as integrated components of the plan.
• Reviews admissions and alive discharges to ensure eligibility and appropriateness.
• Collaborates with team and attending physicians on palliative care measures.
• Ensures complete and accurate data for recertification decisions and timeliness.
• Reviews imminent alive discharges to assure effective discharge planning.
• Monitors service levels and coordinates 24/7 team coverage.
• Manages team schedule and on-call rotation participation.
• Facilitates team meetings with interdisciplinary input into the care plan.
• Engages in quality improvement, regulatory compliance, and staff development.
