Job summary

Capital Medical Group seeks an APP to join the inpatient Palliative team. The APP will provide high-quality, patient-centered care to hospitalized patients while collaborating with physicians and an interdisciplinary team.

You will evaluate and manage patients throughout their stay, participate in rounds, develop treatment plans, coordinate care, and facilitate transitions of care. Responsibilities include education for patients and families, documentation, and participation in

Qualifications

• Graduate degree from an accredited Nurse Practitioner program.
• Current national certification appropriate to the provider's profession.
• Current or eligible licensure in the applicable jurisdiction.
• Prescriptive authority and controlled-substance registration as required.
• Current or eligible DEA registration, as applicable.
• Ability to meet all organizational employment, occupational health and background-screening requirements.

Responsibilities

• Perform comprehensive patient assessments, histories, and physical examinations.
• Evaluate and manage hospitalized patients in collaboration with physicians and the care team.
• Develop, implement, and modify treatment plans based on clinical findings.
• Order and interpret diagnostic tests, medications, and treatments within scope.
• Participate in daily rounds and multidisciplinary care planning.
• Coordinate admissions, transfers, and discharge planning.
• Provide education and support to patients and families regarding diagnoses and follow-up care.
• Maintain accurate clinical documentation and complete billing/charge-capture activities.
