Qualifications

• Assesses patient physical, psychological, social, spiritual and discharge planning needs.
• Plans care in collaboration with the multidisciplinary team and develops realistic goals.
• Implements plan of care, delegates tasks and coordinates nursing activities.
• Documents assessment, planning and evaluation in the patient record.
• Promotes evidence-based practice and supports teamwork and education.
