Qualifications

• Active Clinical Pharmacist license in the state of Illinois (IDFPR)
• Bachelor’s Degree in Pharmacy or a Doctor of Pharmacy Degree

Responsibilities

• Interprets medication orders to ensure safety and proper use of medications; coordinates solutions across pharmacy, nursing and prescribers.
• Monitors therapy for significant drug interactions; intervenes when necessary to prevent adverse outcomes.
• Evaluates and documents appropriateness of selected medications for therapeutic outcomes; communicates with prescriber.
• Oversees dispensing processes and ensures accurate documentation in the Electronic Health Record.
• Educates patients and healthcare professionals about medications and participates in training programs.
• Participates in quality improvement and formulary management activities.
