Job summary

Dana-Farber Cancer Institute seeks a Care Coordinator to support hospitalized oncology patients at Mount Auburn Hospital. You will coordinate non-clinical care, navigate the healthcare system, and reinforce care plans with the clinical team.

This role emphasizes collaboration with physicians, nurses, social workers, and community services; you will identify social determinants of health, document in the EHR, and help families manage appointments and referrals.

Qualifications

• Active Massachusetts RN license required.
• 1 year of care coordination or related experience preferred.
• Associate's degree required; bachelor’s degree preferred.

Responsibilities

• Serve as primary non-clinical point of contact for a panel of high-risk patients and families.
• Coordinate appointments, referrals, and follow-up across primary care, specialty, and community services.
• Identify social determinants of health and connect patients to resources.
• Collaborate with interdisciplinary care team to align services with patient goals.
• Provide education reinforcement and reminders using plain language.
