Carolinas Rehabilitation NorthEast in Concord, NC seeks a qualified therapist to evaluate and direct physical therapy, occupational therapy, or speech-language pathology programs for adult patients recovering from complex conditions. The role emphasizes interdisciplinary collaboration and patient-centered care.

Responsibilities include planning, implementing, and documenting therapy plans, coordinating with physicians and staff, and supervising Rehab Techs and assistants.

Qualifications

• Bachelor's degree in Physical Therapy, Occupational Therapy, or Master's in Speech-Language Pathology.
• Applicable state license for PT/OT/SLP required.
• Basic Life Support for Healthcare Provider certification required.

Responsabilités

• Provide direct therapy services to patients 7 days a week, including holidays.
• Evaluate patients and develop individualized care plans.
• Document evaluations, tests, progress notes, discharge notes and related records.
• Communicate evaluations and progress to physicians and clinical staff; coordinate scheduling and discharge planning.
• Supervise PTA, COTA and Rehab Tech/Aide as required for licensure.
