Mount Sinai is seeking a Social Worker for inpatient Med/Surg to deliver psychosocial services with a focus on complex discharge planning for patients on 9 East, including medical and GI surgery cases.

You will collaborate with an interdisciplinary team to support patients and families facing acute or chronic illness, and help connect resources for treatment, end-of-life planning, and transitions of care.

Qualifications

• NYS LMSW/LCSW.
• Excellent clinical, collaborative, and organizational skills.
• Experience in providing psychosocial services to patients with acute and chronic illnesses.
• Ability to develop and implement safe, appropriate discharge plans in a demanding, fast-paced environment.
• Ability to lead in coordinating multiple disciplines for complex family meetings.
• Knowledge of crisis, family, and systems theories.
• Inpatient hospital experience preferred.
• Internal candidates: Minimum one year in current position unless a Float in Medicine.

Responsabilités

• Provide psychosocial services with emphasis on complex discharge planning for patients on 9 East.
• Collaborate as a member of the interdisciplinary team to support patients and families.
• Linkage to substance abuse treatment resources and end-of-life planning.
• Coordinate crisis intervention and family meetings across units.
