Montefiore Medical Center is seeking a Social Worker to join an interdisciplinary team in the Moses Division. The role focuses on identifying psychosocial problems, coordinating discharge and treatment plans, and collaborating with patients, families and the clinical team to ensure safe, timely care.

The position requires an MSW and NYS LMSW or LCSW licensure, with experience in social work practice standards and confidentiality under NASW ethics.

Competenze

• Master of Social Work (MSW) required.
• LMSW or LCSW licensure by the NYS Board of Social Work.
• MSW from an NYS registered school or substantially equivalent program.

Mansioni

• Screen patients for social service needs per practice standards.
• Perform psychosocial assessments using patient/family input and team input.
• Ensure timely discharge/treatment plans with patient, family and team.
• Work independently while using supervision to address discharge/treatment delays.
• Maintain confidentiality per Montefiore policies and NASW ethics.
