Job summary

Qualifications

• Minimum of two years of experience in acute health care or community health settings.

Responsibilities

• Provide direct clinical social work services, including counseling, discharge planning, and case management.
• Support inpatient hospital units and review daily census for interventions.
• Conduct biopsychosocial assessments addressing emotional, psychological, social, financial, and environmental needs.
• Develop and monitor individualized care and discharge plans.
• Collaborate with physicians, nurses, and interdisciplinary teams; triage requests at VacaValley campus.
• Facilitate coordination with community resources for optimal outcomes.
• Communicate patient information across the continuum of care for continuity.
