Job summary

Northwestern Memorial Hospital seeks a licensed clinical social worker (LCSW) to provide comprehensive psychosocial assessments, discharge planning, and resource coordination for diverse patient populations across our campus.

You will collaborate with care teams to implement care plans, offer crisis intervention, and advocate for patients and families while maintaining compliance with policies and ethical standards.

Qualifications

• Licensed Clinical Social Worker (LCSW) licensure required.
• Master's Degree in Social Work (CSWE-accredited).
• Minimum of two years post-graduate hospital Social Work experience.
• Strong interpersonal and communication skills.
• Ability to prioritize and manage caseload; self-directed work.
• Analytical skills to analyze data and negotiate in sensitive situations.

Responsibilities

• Conduct psychosocial assessments and intervene with comprehensive care plans.
• Coordinate discharge planning and resource utilization with the care team.
• Provide crisis intervention and protective services when needed.
• Support patient and family education and advocacy throughout hospitalization.
• Collaborate with multidisciplinary teams to optimize patient outcomes.
