Qualifications

• MSW from a CSWE-accredited program.
• California LCSW licensure (CCS paneling for NICU/pediatric population if applicable).
• Minimum two years’ experience in acute care or community health settings.

Responsibilities

• Provide direct clinical social work services, including counseling, discharge planning, and case management for hospital patients and families.
• Perform biopsychosocial assessments addressing emotional, psychological, social, financial, environmental, and follow-up referral needs.
• Support ED mental health cases, including safety planning.
• Develop care plans and monitor progress across the continuum of care.
• Collaborate with physicians, nurses, case managers, and interdisciplinary teams for coordinated care.
• Document assessments, interventions, and care plans in the EMR.
