Job summary

Qualifications

• Licensed Clinical Social Worker (LCSW) licensure required in Illinois.
• Master's Degree in Social Work from CSWE-accredited program.
• Minimum of two years post-graduate hospital social work experience.

Responsibilities

• Meet with patients and families to assess social, emotional, cultural and financial circumstances and develop intervention plans.
• Provide psychosocial interventions addressing illness, decisions, discharge planning and post-hospital needs.
• Coordinate discharge planning with interdisciplinary team to ensure timely and safe transitions.
• Assist with resource utilization, insurance questions and community referrals.
• Document activities accurately in medical records and systems.
