Job summary

Crossing Hurdles is seeking a seasoned inpatient coder to review and analyze medical records for accurate ICD-10-CM/PCS coding. You will validate diagnoses, POA indicators, MS-DRG assignments, and flag hospital-acquired conditions, while comparing AI-generated notes to documentation.

This role requires active coding certification and ongoing CDI expertise to ensure coding integrity across records.

Qualifications

• Active certification in CIC, CCS, or AHIMA/AAPC.
• Strong experience in inpatient ICD-10-CM/PCS coding.
• Access to up-to-date ICD-10-CM/PCS coding books and Coding Clinic guidance.
• Currently working in inpatient medical coding or CDI.

Responsibilities

• Review and analyze inpatient medical records for accurate ICD-10-CM/PCS coding.
• Review clinical documentation for accuracy, completeness, and coding integrity.
• Identify principal and secondary diagnoses, sequence codes, and POA indicators.
• Validate MS-DRG assignment and flag HACs and patient safety indicators.
• Evaluate AI-generated clinical notes against real-world documentation and coding standards.
• Identify and resolve discrepancies in diagnosis and procedure coding.
