Job summary

Crossing Hurdles is seeking a qualified medical coder to review inpatient records and ensure accurate ICD-10 and CPT coding. You will assess clinical documentation for completeness and coding integrity, and evaluate AI-generated notes against established standards.

Applicants should hold AHIMA/AAPC coding certification and have strong ICD-10 experience. The role involves coding AI-generated information and entering data into electronic medical records.

Qualifications

• Active certification in CCS, CIC, COC, RHIA, RHIT, or other coding certification through AHIMA and AAPC.
• Strong relevant experience in ICD-10 coding.
• Access to up-to-date official ICD-10 and CPT coding books, as well as E/M guidelines.
• Currently working in medical coding, clinical documentation improvement, or a closely related documentation role.

Responsibilities

• Review inpatient medical records to ensure accurate ICD-10 and CPT coding.
• Review clinical documentation for accuracy, completeness, and coding integrity.
• Evaluate AI-generated clinical notes and summaries against real-world documentation and coding standards.
• Code AI-generated diagnostic and procedural information using ICD and CPT/HCPCS classification systems.
• Identify and resolve discrepancies in diagnosis and procedure coding.
• Enter AI-generated patient information into computerized medical record databases.
