Qualifications

• Active Ohio Pharmacist License in good standing.
• Inpatient hospital experience required.
• Experience with EPIC preferred.

Responsibilities

• Order verification and clinical review.
• Medication dispensing and distribution.
• Medication therapy monitoring, including drug interactions and dosing.
• Pharmacokinetic dosing and clinical support.
• Medication reconciliation and discharge counseling.
• Collaboration with providers and healthcare staff.
• Maintain accurate documentation and compliance with pharmacy regulations.
