Qualifications

• Active Pharmacist license in state of practice.
• Hospital pharmacy experience preferred.
• Strong clinical judgment and attention to detail.
• Excellent communication and interdisciplinary collaboration skills.
• Ability to work independently in a fast-paced inpatient environment.

Responsibilities

• Review and verify medication orders for accuracy and appropriateness.
• Support safe and effective medication therapy for hospitalized patients.
• Collaborate with physicians, nurses, and healthcare teams on medication-related decisions.
• Monitor for drug interactions, dosing concerns, and medication safety issues.
• Provide drug information and clinical support to patient care teams.
• Ensure compliance with pharmacy regulations, policies, and quality standards.
• Document pharmacy activities within the electronic medical record.
