Job summary

Beth Israel Lahey Health seeks an experienced Hospice RN to provide comprehensive nursing care for hospice patients in various settings within Massachusetts. You will work within an interdisciplinary team to develop and execute patient-centered care plans, manage symptoms, and educate families on medications and comfort measures.

The role requires MA RN licensure, experience in hospice or home health, and proficiency with PTCT EMR.

Qualifications

• Current Massachusetts RN license.
• BSN preferred.
• Hospice Case Management or related Home Health experience preferred.
• EMR (PTCT) experience in hospice documentation.
• Competence in pain and symptom management, IV/SC infusions, compounded medications.

Responsibilities

• Provide care according to hospice philosophy.
• Interact with interdisciplinary team and family to develop patient-specific plan of care.
• Communicate patient condition and guidance on end-of-life pain and symptoms to physician.
• Perform RN pronouncements outside acute care hospital.
• Document assessments and updates in EMR (PTCT).
• Coordinate care with attending physician, Hospice Medical Director, pharmacy, and IDG to ensure optimal patient care.
• Educate patient and family on medications and care processes to maximize comfort.
• Ensure plan of care is updated and reviewed with IDG every 14 days and prior to recertification.
