Beth Israel Lahey Health is hiring a Hospital Based Admissions Nurse for hospice admissions coordination, based out of Mount Auburn Hospital in Watertown, MA. This full-time, day-shift role offers 40 hours weekly with no weekends and requires MA RN licensure and BLS.

The position handles preadmission activities, eligibility assessments, and collaboration with the care team to ensure smooth hospice admissions and ongoing patient care. Salary ranges from 87k to 128.9k USD depending on experience.

Qualifications

• MA RN license with NLN-accredited program.
• BSN strongly preferred.
• Current BLS certification.
• Minimum 3 years hospice experience.
• Inpatient hospice experience preferred.
• Excellent communication and teamwork.

Responsibilities

• Assess and coordinate GIP level of care admissions.
• Explain hospice services to patients/families.
• Provide care planning with care team and physician.
• Manage patient care and symptom control per plan.
• Document with EMR and HOPE measures.
