Job summary

You will review medications, counsel patients, collaborate with physicians, and support a care team while ensuring compliance with laws and regulations. This role requires a PharmD or PharmD and up to 2 years experience; licensure in MN and WI may be required.

Qualifications

• Bachelor's degree in Pharmacy or PharmD from an accredited school.
• 0 to 2 years of identified pharmacy experience.

Responsibilities

• Reviews medications and patient profiles for interactions.
• Evaluates dosage, route, frequency, effectiveness, and cost.
• Dispenses medications and IVs; ensures accuracy.
• Provides patient counseling and consultation.
• Collaborates with physicians and care teams to optimize therapy.
