Qualifications

• Bachelor's degree in Pharmacy or PharmD from an accredited school.
• Completion of a residency program is preferred.
• Board certification is preferred.
• 2+ years of pharmacy experience is preferred, including hospital/home infusion exposure.

Responsibilities

• Reviews medications and patient medication profiles for interactions and appropriateness.
• Dispenses medications, enters and verifies orders, and prepares IVs.
• Provides counseling and education to patients on medications and use.
• Collaborates with physicians and health-care teams to optimize therapy.
• Manages home infusion patient care plans and coordinates with care partners.
