Qualifications

• Bachelor's degree in Pharmacy OR PharmD from an accredited school of pharmacy.
• 0 to 2 years of identified pharmacy experience.

Responsibilities

• Reviews medications and patient profiles for interactions and appropriateness.
• Dispenses medications and verifies accuracy of preparation and labeling.
• Provides counseling and consultation to patients and healthcare teams.
• May educate patients on medications and collaborate with home infusion partners.
• Provides direction and training to Pharmacy team as needed.
