Qualifications

• Licensed pharmacist in MN or WI preferred upon hire.
• Experience with home infusion pharmacy and IV admixture is advantageous.
• Strong knowledge of drug interactions, dosing, and formulary principles.

Responsibilities

• Reviews medications and patient profiles for interactions and appropriateness.
• Dispenses medications and verifies preparation and labeling.
• Provides counseling and pharmacotherapeutic recommendations to optimize therapy.
• Leads pharmacy team activities and coordinates with care partners in home infusion.
