Job summary

UVA Community Health, part of the UVA Health system, seeks a Clinical Pharmacist to collaborate with the healthcare team to provide individualized care and optimize therapy for patients across UVA Health’s community locations.

The pharmacist will participate in assessments, plan and implement patient care, and ensure adherence to policies while maintaining high standards of medication safety and quality. On-call and travel to multiple sites may be required.

Qualifications

• 4 Year/Bachelor's Degree in Pharmacy required.
• FPGEC Certification required or eligible.
• 1 year of experience or PGY1/2 in health system pharmacy.
• Virginia Pharmacist license required within 30 days of start date.

Responsibilities

• Provide medication management and safety.
• Participate in patient care planning and rounding.
• Monitor therapy for safety, effectiveness, and interactions.
• Maintain documentation and compliance with policies.
