St. Joseph Hospital in Nashua, NH seeks an Inpatient Clinical Pharmacist to provide comprehensive pharmaceutical care for hospitalized patients.

You will review medication orders, verify and dispense medications, and work with physicians, nurses, and the interdisciplinary team to optimize therapy. You will monitor pharmacotherapy, perform reconciliations, counsel patients and families, and contribute to antimicrobial stewardship, formulary management, and adherence to regulations and hospital

Qualifications

• DPharmD degree from an accredited institution; licensure as a Pharmacist in NH.
• Preference for completion of a PGY1 residency in inpatient clinical setting.
• Strong inpatient pharmacotherapy management and medication reconciliation skills.
• Proven ability to collaborate with physicians, nurses and other care team members.

Responsabilités

• Review and verify medication orders for hospitalized patients.
• Dispense medications and ensure safe and effective therapy.
• Collaborate with physicians and nurses to optimize pharmacotherapy.
• Participate in clinical rounds and provide evidence-based recommendations.
• Monitor pharmacotherapy and perform medication reconciliation.
• Counsel patients and families on medications and therapy plans.
• Respond to drug information inquiries from staff and patients.
• Support quality improvement initiatives, antimicrobial stewardship and formulary management.
• Ensure compliance with hospital policies, accreditation standards, and applicable laws.
