Job summary

St Joseph Hospital in Nashua, NH seeks a Clinical Pharmacist to provide medication distribution and clinical drug information across the hospital and to the care team under the Director of Pharmacy.

The role includes daily chart reviews, optimizing therapy for high‑risk meds, counseling patients at discharge, and collaborating with physicians, nurses and other clinicians in centralized and decentralized settings.

Qualifications

• Current NH license to practice pharmacy.
• Pharm.D. or higher preferred.
• BCPS preferred.
• 1–3 years clinical pharmacy experience preferred.
• Residency training or direct patient care experience preferred.
• Experience on multidisciplinary teams preferred.

Responsibilities

• Performs any task required to provide safe and efficient medication therapy to patients.
• Monitors the administration of medications and verifies dosing, interactions and allergies.
• Performs daily clinical chart reviews for assigned shift and documents appropriately.
• Creates interventions to decrease readmission risk and improve medication safety.
• Provides updated drug information to providers and staff with cost-effective recommendations.
• Collaborates with physicians, nurses, and other clinicians across hospital systems.
• Supervises Pharmacy Technicians and serves as a preceptor for students.
• Participates in performance improvement projects and hospital CQI activities.
