Job summary

UCSF Health is seeking a Clinical Staff Pharmacist II to deliver comprehensive pharmaceutical care across inpatient settings within the UCSF Health System. You will help ensure safe, effective medication use, review patient histories, and support clinicians with therapeutic plans while maintaining high standards of patient safety.

The role requires a PharmD, California license, and BLS certification, with ASHP PGY1 residency or 3+ years of acute care experience preferred.

Qualifications

• PharmD degree or equivalent as determined by state board.
• Current California license in good standing to practice pharmacy.
• Current Basic Life Support (BLS) certification.
• ASHP-accredited PGY1 Residency or 3+ years inpatient/acute care experience (preferred).
• Advanced practice board certification (e.g., BPS, AAHIVP) (preferred).
• Experience participating in pediatric and adult emergency situations (preferred).

Responsibilities

• Provide pharmaceutical care to patients across UCSF Health System inpatient locations.
• Review medication profiles, obtain histories, and provide drug information to patients and clinicians.
• Collaborate with clinicians on drug choices and therapeutic plans; ensure access and safe use of medications.
• Educate patients on proper medication use and potential adverse reactions; oversee pharmacy technicians as needed.
