Job summary

The Chickasaw Nation Department of Health is seeking a full-time Inpatient Case Manager to join the Inpatient Case Management and Social Work Team at Chickasaw Nation Medical Center. 8:00 AM–5:00 PM, Monday through Friday, with responsibilities for care coordination, utilization review and discharge planning.

Qualifications

• Graduate from an accredited school of nursing and be licensed as RN.
• BLS certification is required.
• Minimum of 2 years of related nursing/case management experience.
• Maintain confidentiality and HIPAA compliance in all tasks.

Responsibilities

• Oversee daily care coordination and plan patient services within guideline/protocols.
• Coordinate information exchange with outside facilities while protecting PHI.
• Develop and implement continuity of care and discharge planning.
• Assess patients’ needs, resources, and eligibility for funding sources.
• Utilize chart reviews and EHR documentation for admissions and reviews.
• Support documentation of appropriate level of care and referrals.
• Collaborate with hospital staff to ensure safe transitions and appropriate follow-up.
• Serve as liaison with providers, payers, and community resources to optimize care.
• Assist in data collection for performance improvement and reporting.
• Maintain professional relationships with Chickasaw Nation departments and clinical teams.
