Qualifications

• Education and Training: Licensure in the state where the hospital resides is preferred.
• Professional disciplines may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or licensed social worker.
• Current BLS certification required.

Responsibilities

• The Case Manager is responsible for the interdisciplinary coordination of care for a designated patient population.
• Performs reviews of inpatient admission records to ensure proper utilization and determine admission level of care.
• Assesses discharge needs and coordinates the discharge plan with the interdisciplinary team.
• Works to facilitate achievement of financial and quality outcomes and maintains payor relationships.
