Job summary

PAM Health Rehabilitation Hospital of Venice is seeking a Case Manager to coordinate interdisciplinary care for a designated patient population and oversee admission determinations and discharge planning. The role involves working with a broad care team to ensure appropriate resource use and successful patient outcomes.

Ideal candidates have 3–5 years inpatient experience, licensure in the state where the hospital resides (preferred), and current BLS certification.

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Current BLS certification required.
• Three to five years of inpatient experience is expected.

Responsibilities

• Coordinate interdisciplinary care for designated patient population.
• Review inpatient admission records to ensure appropriate utilization and level of care.
• Identify discharge needs and coordinate the discharge plan with the team.
• Maintain and enhance payor relationships.
