Job summary

Responsibilities

• Coordinate and manage patient care throughout the acute inpatient episode.
• Ensure continuity of services and appropriate utilization of resources.
• Document assessments, interventions, and care plans to improve care continuity.
• Escalate needs to the facility case management coordinator, clinical advisor, or director of care management.
• Demonstrate leadership and participate in quality improvement initiatives.
