The position requires an IL RN license with BSN preferred and 2–5 years in case management, CPR, and hospital experience.

Qualifications

• Currently licensed as an Registered Nurse in Illinois.
• BSN preferred; 2–5 years case management experience preferred.
• Current CPR certification.
• Relevant hospital nursing; hospital case management; insurance case management experience preferred.

Responsibilities

• Monitors and coordinates care and hospitalization plans.
• Completes continuity of care assessments and discharge planning.
• Collaborates with multidisciplinary team to facilitate care transitions.
• Delivers CMS regulatory notices within required timeframes.
• Promotes patient privacy and confidentiality.
• Maintains safe environment and uses error prevention habits.
• Provides education as needed to staff.
