Job summary

Carle Health is seeking an experienced RN Case Manager to oversee the clinical and financial outcomes for acutely ill hospitalized patients. You will coordinate assessments, discharge planning, and transition to appropriate post‑hospital care, ensuring timely, safe patient journeys.

In this role you will work with physicians, social workers, and care teams to manage length of stay, reduce readmission risk, and document care plans. Bachelor's degree and Illinois RN license required.

Qualifications

• Licensed Registered Nurse (RN) in Illinois (IDFPR)
• Bachelor's Degree required

Responsibilities

• Act as liaison with patient/family and physician to determine next level of care and manage LOS.
• Coordinate transition from inpatient to post-hospital care with physicians, consultants, and referrals to meet patient goals.
• Document plan of care and utilization issues in case management and multidisciplinary records.
• Evaluate effectiveness of plans and collaborate with physicians to reduce readmission risk and LOS.
• Investigate coverage for post-hospital needs and present options to patient/family and provider.
• Participate in multidisciplinary rounds and initiate referrals to quality improvement, risk management, social work, and other teams.
