Job summary

Carle Health is seeking an experienced RN Case Manager to oversee clinical and financial outcomes during acute hospitalization. You will assess patients, coordinate discharge planning, and ensure a safe transition to the next level of care.

Qualifications

• Licensed Registered Nurse (RN) in Illinois (IDFPR).
• Bachelor's Degree in a related field.

Responsibilities

• Act as liaison with patient/family and physician to determine next level of care and manage LOS.
• Coordinate transition from inpatient care to post-hospital care with physicians, consultants, internal and external referrals to meet patient goals.
• Document plan of care and utilization in case management software and the multidisciplinary care plan document.
• Evaluates effectiveness of plan of care and collaborates with physicians to reduce risk of readmission and length of stay.
• Initiate intervention for patients identified from a proactive admission assessment and referrals from healthcare team.
• Participate in multidisciplinary rounds and coordinate timely referrals to social work, therapy services, and other team members.
