Qualifications

• Currently licensed as a Registered Nurse in the state of Illinois.
• BSN preferred.
• 2-5 years previous Case Management experience preferred.
• Current CPR.
• Relevant hospital nursing; hospital case management; insurance case management experience preferred.

Responsibilities

• Monitors, coordinates and evaluates the care and appropriateness of hospitalization; initiates referrals.
• Completes Continuity of Care assessments and establishes appropriate plans by initiation of timely interventions through collaboration with patients, families and multidisciplinary care team respecting choice of patient.
• Collaborates with multi-disciplinary team and informs them of variance from established care pathways to ensure timely progression of care and eliminates barriers for next site of care transitions.
• Assists in expediting care delivery to avoid delays in timely service provision.
• Provides feedback to medical staff and conduct education as expert resource regarding care coordination, discharge planning needs and regulatory requirements.
