Job summary

Qualifications

• Graduate of a Board of Nursing approved nursing education program (RN).
• BSN degree preferred.
• Typically requires 3 years of related nursing experience.
• Case management experience preferred.
• Professional certification preferred within 3 years of hire.

Responsibilities

• Conducts complete assessments, establishes appropriate plans, and initiates interventions within desired timeframes.
• Provides continuity of care and discharge planning per standards and regulations.
• Provides case management across health care, finances, housing, family issues.
• Initiates referrals to ensure timely progression of care and transitions.
• Communicates effectively with the healthcare team and manages UM activities.
• Collaborates with managers, physicians, medical directors, advisory groups, and treatment teams on patient care plans.
• Keeps current with healthcare regulations and CMS notices; delivers updates as needed.
• Develops relationships with community agencies and represents Advocate Health Care.
• Educates staff on admission status, UM issues, and discharge planning needs.
• Demonstrates age-specific care knowledge and development considerations.
