Job summary

Triple-S Management Corp. is seeking an Operations Coordinator to support the Utilization Management and Transition of Care teams. You will manage inpatient census data, ensure timely entry, and aid daily clinical workflow within a health plan setting.

The role emphasizes data integrity, accurate reporting, and coordination of post-discharge activities with PCPs, specialists, and vendors to promote safe transitions and high-quality member care.

Qualifications

• Bachelor’s Degree with six months of experience in similar activities, or Associate degree with at least two-three years of experience in operational support activities.
• Experience within a health plan organization, hospital admissions or care coordination is preferred.

Responsibilities

• Daily Inpatient Census Management: Data Entry and Reconciliation of census updates from facilities.
• Status Tracking: monitor discharges, transfers, changes in level of care, and readmissions for accurate length-of-stay tracking.
• Clinical Support Escalation: identify and notify clinical team members of new admissions or priority cases requiring concurrent review; facilitate management reports.
• Transition of Care (TOC) Operational Support: collect, verify, and upload post-discharge documentation; verify completeness; schedule follow-up appointments.
• Communication, Quality, & Compliance: serve as liaison among health plan, discharge coordinators, and providers; log interactions and data transmissions; propose workflow improvements.
