Job summary

UT Southwestern Medical Center is seeking a Medical Social Worker (LMSW) in Inpatient Care Coordination. This 100% onsite role requires a Texas LMSW license and typically involves discharge planning, patient education, and collaborative care transitions. Schedule is Monday-Friday with daytime hours.

The Care Coordination Department values psychosocial assessment, team collaboration, and coordinated transitions to post-acute care. Two years of hospital experience is preferred.

Qualifications

• Licensed Master Social Worker (LMSW) in Texas.
• 2 years hospital experience.

Responsibilities

• Screens and evaluates high risk patients for discharge planning needs.
• Consults with attending physicians and healthcare team regarding psychosocial issues and care transition barriers.
• Collaborates with the multidisciplinary team to establish and execute the discharge plan.
• Facilitates patient care conferences for complex cases to establish a safe discharge plan.
• Implements the transition of care plan with referrals and ensures patient choice in services.
• Assists with abuse and neglect cases, including assessment and referrals as appropriate.
• Educates patients and families providing information and resources.
