Job summary

The University of Texas Southwestern Medical Center in Dallas is seeking a Medical Social Worker (LMSW) to serve as an Inpatient Care Coordinator within the Care Coordination Department. You will educate the healthcare team on psychosocial issues and plan transitions of care.

Responsibilities include screening high-risk patients for discharge planning, collaborating with the multidisciplinary team, leading care conferences, and guiding referrals to post-acute services.

Qualifications

• LMSW licensure in Texas is required.
• Minimum two years hospital experience.
• Strong discharge planning and care coordination skills.
• Ability to educate multidisciplinary teams on psychosocial issues.

Responsibilities

• Screens and evaluates high-risk patients for discharge planning.
• Collaborates with physicians and care team to address psychosocial barriers.
• Leads care conferences for complex discharge planning and transitions.
• Executes transition of care plans and referrals to post-acute services.
• Assists with abuse/neglect cases and connects to appropriate agencies.
• Educates patients/families about community resources and payor benefits.
• Maintains patient records and notes related to social work interventions.
