Qualifications

• Requires LMSW license in Texas.
• Minimum 2 years hospital experience preferred.
• Experience with discharge planning and care coordination.

Responsabilités

• Screen and evaluate high risk patients for discharge planning needs.
• Collaborate with physicians and healthcare team on psychosocial issues.
• Facilitate discharge planning conferences and coordinate care transitions.
• Coordinate referrals and community resources for patients and families.
• Maintain patient records and document care transitions and outcomes.
